I, ______________________, hereby acknowledge that my participation in activities at First Creek Fight Club involves inherent risks, including but not limited to physical injury.

In consideration of being allowed to participate, I agree to assume all risks associated with the activities and hereby release First Creek Fight Club, its employees, and affiliates from any and all liability for injuries or damages that may occur.

I certify that I am physically fit to participate in these activities and have no medical conditions that would jeopardize my/others safety. In case of an emergency, I authorize the staff to seek medical attention on my behalf.

I understand the risks associated with exposure to blood borne pathogens (such as HIV, Hepatitis B, and Hepatitis C) during MMA activities. I agree to adhere to all safety protocols and report any injuries or situations where blood contact may occur.

I understand and agree that this waiver is binding on me and my heirs. I have read and voluntarily signed this waiver with the understanding of its content.

A.  I consent and authorize First Creek Fight Club, located at 16431 US-72, Rogersville, AL 35651 (“Releasee”), to use: (Check one)

B.  I grant the Releasee the irrevocable right to use the Photos for lawful commercial purposes, including but not limited to advertising, promotion, marketing, or trade, in any format or publication, whether print or web- based.

☐  I represent that I am at least 18 years of age and have the full legal capacity to execute this Photo Release Form.

E.  This agreement shall be governed by the laws of the State of Alabama, and any disputes arising from this agreement shall be resolved in the courts of the State of Alabama .

F. If any provision of this Photo Release Form is found to be invalid or unenforceable, the remaining provisions shall continue in full force and effect.
